The Physician serves as the Primary Care Physician (PCP) Director for the Santa Rosa Community Based Outpatient Clinic located in Santa Rosa, CA. The physician will have both administrative duties and direct patient care duties with 30% of his or her time devoted to administrative duties as Santa Rosa CBOC Medical Director and 70% devoted to direct patient care. The Medical Director leads provider teams at this site and report to the Director of Primary Care for Medical Service and provides comprehensive care for our Veteran population.
Supervisory functions include but (not limited to) supervision of providers, ensure adequate staffing, leave approval, ensure and guide the clinic staff/providers to meet various clinical performance measures and access requirements, assist the Director Primary Care for Medical Service and facility leadership meet various VHA requirements, assist in meeting various regulatory agencies' (like OIG, JC and others) standards.
Provides primary care services in a team based setting, including chronic disease management and preventive health.
Leads the PACT team in coordinating care of the veterans in non-face to face or group settings with team members.
Evaluates, diagnoses, treats, and provides consultation for non-surgical adult patients of all ages, without life-threatening complications and with a wide variety of diseases, injuries, and disorders.
Assesses, stabilizes, and determines disposition of patients with emergent conditions consistent with medical staff policy regarding emergency and consultative call services.
Collaborates with /oversee mid-level providers such as nurse practitioners, and works collaboratively with other specialties (and subspecialties) in and outside of the VA system to manage or co-manage patients' health.
Obtains patient histories, perform physical exams, formulate a working diagnosis and develop/implement a treatment plan according to clinical guidelines.
Determines appropriate hospitalization, and/or disposition to Nursing Home Care, Assisted Living, Community Nursing Home, and/or Home Care.
Orders lab tests, x-rays, EKG's and other diagnostic procedures.
Initiates or expedites requests for consults, special tests, or studies provide education and counseling to patient and family in preventive care, medical conditions and use of prescribed treatment and drugs.

Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Completion of a first-year residency, or its equivalent, approved by the Department of Veterans Affairs for the year in which it was completed.
Proficiency in spoken and written English.
